Project Summary

The Project is a 3 years “Ageing-in-place–The Programme for Building a Dementia-friendly Community” (Basic Version) Project under the Dedicated Funding Application. It targets to ride on the collaboration of the community, welfare, business, education and medical sectors to carry out the Project in Tai Hang Tung Estate, Nam Shan Estate, Mei Yue House and Mei Ying House of Shek Kip Mei Estate in Sham Shui Po District. The Project aim at enhancing public awareness of dementia through activities such as street promotion booths, education talks and life story workshops. With the collaboration with NGO and medical organization, the Project organize activities and mutual support groups for the elderly affected by dementia and their carers, in order to enhance their sustainable participation and to build the community support networks. The Project will use the “House Captain” scheme and “Time Bank” to facilitate the active involvement of the members in the community in weaving the community support network. Moreover, the Project will establish cross sectors platform, utilize the advantages and strengths of each stakeholder and linking resources of different sectors to build a Dementia-friendly community with collaborative effort.

The project was funded by the Community Investment and Inclusion Fund in October 2020 for three years.

 

Project Objectives

  1. To enhance community understanding of the needs of the elderly and their families.
  2. To promote community participation of the elderly and their carers and enhance their ability to age in the community.
  3. To build a community mutual help network to support the elderly in need and their families.
  4. To leverage the advantages and strengths of community members and collaborators to support. the elderly and their families, so as to build a friendly community for the elderly to age in place.

Target Group

Elderly and their families living in Tai Hang Tung Estate, Nam Shan Estate,Mei Yue House and Mei Ying House of Shek Kip Mei Estate

Project Content

  1. Enhance community understanding of the needs of the elderly and their families through community education and experiential learning activities.
  2. Organize appropriate activities for elderly, elderly with mild cognitive impairment (MCI) and their carers to enhance their community participation, develop their self-confidence and knowledge about the community resources in order to enhance their capability to age in the community.
  3. Organize mutual support groups for the elderly, elderly with mild cognitive impairment (MCI) and their carers, in order to build the peers mutual support network.
  4. Recruit and provide training to “Community Guardians” and “House Captains” in order to provide community support service to the elderly in need and their families.
  5. Establish the “Time Bank” to enhance active community participation to serve the neighbors and to build a community support network for the elderly and their families.
  6. To build collaborative platform for all the collaborators and to utilize their advantages and strengths to response to the needs of the elderly and their families.
